Navigate to the MyLeave login page

Click "I've forgotten my Worker Registration Number"

Click the "Email" field and add your email address
4
Click the "Date of Birth" field and add your date of birth
5
Click the "Enter the code from the image" field and enter the code shown above

Click "Submit"

If you have entered a valid email address, you will receive an email containing your registration number. If you do not receive an email then please contact MyLeave on (08) 9476 5400.
