How To Submit a Kuali-IRB Application

Navigate to <https://usmd.kualibuild.com/research/space/648a3ef7e0996f0208cf30b9>

Click "IRB".

Click the Form "IRB Application"

If this is your first submission, click "Fill out the form".

If this is not your first submission, click "New".

Tip! Once you have created a form, you will see the form listed in your IRB Application Form Page. You can see Draft, In-Progress, and Complete applications. You can only view Application Forms that you have been given access to. For now, this will only be the applications you create during testing.

Select your Submission Type by using the Submission Type drop-down menu. This is how to open an HSRD, 118 Letter Request, or IRB Protocol.

Click "Next" to navigate to the rest of the application.

Once the form is complete, click "Submit".
