How to Submit a Degree Works Adjustment Request
This guide provides a walkthrough for submitting a Degree Works adjustment in Coursedog.

Navigate to [https://app.coursedog.com/#/cm/home](https://app.coursedog.com/#/cm/home)
Proposal Setup

From the drop down menu, select "Degree Works Adjustment for an Individual Student"

Click "Go To Form"
Student Information

Enter the student’s name and Z number.
5
Select the student major.
6
Select the correct college/school.
7
Select the correct academic level. If it is for a graduate student, the form will be routed to Graduate Studies.
Cognate Adjustment

Enter the adjustment in the appropriate field. Please provide as much detail as possible.

Once you are finished entering all information, click submit. The proposal will go to the Program Chair, Dean, Graduate Studies (if for a graduate student) and/or Academic Advising.
