How To Submit A Curriculum Materials Request Form

This guide provides instructions for staff to request new curriculum materials through the Piedmont Unified School District portal. Follow these steps to correctly input course information, specify material quantities, and finalize your submission for administrative review.

Navigate to your school or district webpage. <https://www.piedmont.k12.ca.us/>

Click "Staff Forms" from the Staff header

Click "Curriculum Materials Request Form"

Fill out the "Submitted By Name" field.

Choose your site "Site"

Choose your department. This will help with course selection further down on the form.

Add the email for your department chair. A notification will be sent to this person to review the materials request.

Under Courses, choose the course or courses you would like to order from. If this is a new course, or a course not on the list, choose "New Course"

In the course table, click on the arrow to choose your course. This list will be filtered by the department above so not every course in the district will show :) If it is a new course, choose New Course.

Click on the arrow in the next field to choose the materials for this course. You should see a general description for the materials we have ordered in the past for this course. If this is a new course, choose the New Materials options. you will describe the materials needed later. And the database will be updated for your new course for next year.

Add the quantity you will need for this material. Also consider if any of these materials need to be housed in resource rooms or other places...

Have more materials for this course or another? Add rows and complete the info for each. Don't have any more requests? Delete the extra rows.

In the "Supplemental or other materials" field include any additional details about the instructional materials needed, quantity, publisher. Also include title of course if a new course!

If you have a quote or order form, or a picture of the book you are looking for, upload it here!

This is to flag things for the principal. Anything new/different warrants a conversation with your principal. Check "I've discussed this with my principal" field.

Does this order involve digital licenses> Check Yes or No. This will alert the Tech department.

Click "Submit"!!

You can check on the ordering process in Droplet. Go to "My Submissions"

You'll see all of your submissions here. You can see who it is currently assigned to in the Assigned To column.

In this case, the form is waiting for a fake deptheademail@piedmont.k12.ca.us to approve the request. When you submit a request, an email is sent to the department head you indicated on the form. From there it goes to the principal.
