Click "LOGIN"

Click "First Time Login?"

Enter your "Member ID" or Email Address

Click "Next"

Choose how you would like to receive your verification code from Assisted Rides, and then click "Send Verification Code"

Enter the Verification code you received via Text or Email, and then click "Verify Code"

Enter your new password (twice) and then click "Save New Password"
8
If applicable, you may need to agree to a HIPAA statement
