Completing the 2025-2026 Annual Performance Evaluation Self-Review

This guide provides a comprehensive walkthrough for completing your annual performance self-review in the Cornerstone system. Follow these steps to efficiently manage your goals, reflect on your core competencies, and submit your evaluation for manager approval.

Navigate to <https://uccs.csod.com/samldefault.aspx?ouid=12285>

Scroll down to the **Your Tasks** widget and click "2025-2026 Annual Performance Evaluation Self-Review"

Review the **Overview** instructions. Once reviewed, scroll down and click "Next"

Review the instructions for **Step 1: Goal Review & Finalization**

This step is your opportunity to make final goal progress updates. To update the progress on a goal, click the dropdown arrow next to the title of your goal...

...then click "Manage Goals"

Update your progress percentage by clicking the percentage dropdown...

...then adjust the percentage sidebar

Be sure to click "Save" following any percentage changes

Attachments and comments may also be entered on this page, but are not required. Once your updates are complete, click "Done" to return to the task

Repeat steps 5-10 for any goals that need updating. Once complete, click "Next" to progress to the next step in the task

Review the instructions for **Step 2: Goal Reflection**

Click the text box to begin typing your response(s) to each question

Complete your response(s), then click "Next" to progress to the next step in the task

Review the instructions for **Step 3: CU Job Competency Reflection**

Click the text box below each competency to enter additional comments. Comments are not required, but are strongly encouraged if you believe you demonstrated exceptional performance in a particular area, and/or there were barriers that impacted your ability to demonstrate a competency.

To review the key behaviors associated with each competency, click the dropdown arrow next to the competency definition...

...then click "Details"

Once reviewed, click "Close"

Complete your response(s), then click "Next" to progress to the next step in the task

Review the instructions for **Step 4: Annual Employee Self-Review**

Click the "Select" dropdown to indicate your self-rating

Select either **Too New To Rate**, **Needing Support**, or **Performing**

To review the 2-point rating scale, click the **?** icon

Once reviewed, click "Close"

Click the text box to provide any additional comments. Comments are encouraged to provide your supervisor with context for your rating, and to highlight accomplishments, explain challenges, and note areas of growth.

Complete your response, then click "Next" to progress to the final step in the task

Review the instructions for **Step 5: Annual Performance Evaluation Sign-Off**

To sign-off on your self-review, click the "First and last name" field...

...type your name, then click "Sign"

Additional comments may be provided, but are not required. Once you've completed your sign-off, click "Submit"

Click "Submit review"

Click "Exit"

To access your finalized performance evaluation after your supervisor has completed their review, click the "Performance Review" button on your Welcome Page...

...then click the "My Personal Reviews" tab...

...then click "2025-2026 Annual Performance Evaluation" to download a PDF of your completed review

Your final rating is also displayed in the **Rating** column
