Registering for Saints of De Pere Discipleship Formation

This guide provides a walkthrough for completing the Saints of De Pere Discipleship Formation registration form. It covers how to input family contact details, enroll individual students in their respective programs, and finalize the process with required agreements and payment information.

Scroll through page, click to see the calendar, explore volunteer and discount opportunities, and expand here to read information about the grade levels and classes.

Whenever you're ready, click "Formation Registration and Payment"

Read the notes, then select your parish.

Fill in all required fields.

Type "00012"

Click the "Address 2 if Required (If a blended family, please include the names of all parents)" field.

Use this field to select the total number of children you need to register. If more than 3, start by registering any children in a sacrament year (2nd or 11th) and note the instructions for child 4 and 5.

Complete all fields for each child.

Select the grade your child will be in 2026-27. 2nd grade requires one year of previous formation. 11th grade requires previous formation as well. Please direct unique situations or questions to formation staff.

Please indicate again if the child is in a sacrament year.

Click the "First Name ``` REQUIRED" field. ```

Fill out all required fields for "Child 2", 3, 4, or 5, following specific instructions for children 4 and 5 if necessary.

Click the "I Agree, I have been made aware of the Safe Environment Social Communications Policy for the Diocese of Green" field.

Click the "I agree with the terms and conditions outlined in the Promotional/Media Release form." field.

Click the "Yes, I authorize communication with my minor/individual at risk electronically." field.

Click "Proceed to Payment"

Choose your preferred payment type - credit card or electronic check. Electronic check is better for the parish.

Click "I'm not a robot."

Complete fields and click "Submit"
