How to Submit an Amendment to the IRB Protocol

Navigate to Kuali-IRB and click on the IRB Application Form tab.

Click on the application you wish to submit an amendment for.

Click on the drop-down menu at the top left corner of the page.

The drop-down menu will present each version of the IRB Application. In this case, there is only one version of IRB Application #0292. Click on "+ Create new version" to create a new version of the application.

The new version will create a copy of the existing IRB Application. Along with the existing pages of the application, a new page, "Amendment Application," will appear.

Complete the Amendment Application as it applies to the changes being requested in the current version. Once the Amendment Application is complete, continue to the following pages of the IRB Protocol and edit each section as needed. Be sure to only make the changes that were described in the Amendment Application.

Once the changes are made and the Amendment Application is complete, click "Submit" to send the application to the IRB Office for review.
