Click "Avon Grove School District"

Click "YEAR Season Registration - Avon Grove Middle School"

Click the student's chosen sport field.

Click the "First Name" field.

Enter the student's name

Click the "Last Name" field.

Enter student's last name

Click here.

Click "August 2026"

Click "2026"

Click here.

Select student's birth year

Select student's birth month

Select student's birth date

Select student's gender.

Click the "Age of Student on Last Birthday" field.

Type the Age

Click the "Email" field.

Enter parent email

Click the "Cell Phone" field.

Enter cell phone number

Select student's grade level

Answer this question for your student

Enter the address of the student

Enter Parent or Guardian Information

Enter Emergency Contact Information 1st and 2nd

Enter the PCP information and Upload Section 6 Form

Click here.

Answer Insurance Question; If yes insurance prompt will populate

If you answer Yes to Insurance this section will need to be filled out

Choose to Add additional insurance or decline the insurance

Answer the Health/Medical section; Yes to some questions may require more information

Review all Privacy Policies

Read and Sign Parent and/or Student Sign-offs - Make signature legible

Review Entire Form and then Click "Submit"
