Adding the Field Trip Permission Slip in PSQ

Click "New Post"

Click "Forms / Permissions" from the Post Add-Ons sidebar.

Click "Select an existing form" and "Select Form"

Click "Field Trip Permission Slip"

Add the date the field trip form is due. Please note that the form will be turned off and become unavailable to parents/guardians after the due date. Consider building some extra time into the due date to accommodate last-minute signatures.

Add details in the body of the post and add a subject title.

If you need chaperones, you can also select "Request Volunteers" from the "Post Add-ons" menu on the left side of the screen when creating the post.

Schedule your post in the "Post Now" drop down menu and that's it!

This is what the field trip form looks like for parents/guardians.

This is what the chaperone sign-up looks like from the parent/guardian view.

After it has been posted, you can view the field trip reports and volunteer sign-ups. Click on the gear icon of the field trip post.

Click "Forms" and view report. You will be able to view who has completed the form, and the parent/guardians who have not completed it.
