Navigate to [https://patient-portal.prosperityehr.com/users/sign_in?target_url=/](https://patient-portal.prosperityehr.com/users/sign_in?target_url=/)

Enter in Email Address and Password and hit 'Log In.'

Click "(Switch)" when there are multiple clients using the same email address. This will allow you to access the individual accounts. If you do not see this, please skip to Step 5.

Click "Connect" to access the individual account.

Click "Get Started" to access the patient portal.

Click "Navigate to Home"

This will allow you to view upcoming appointments, complete forms, view statements, make a payment, or update cell phone # for text reminders.
