Click on "Request type".

Choose any of the related options to your issue in the dropdown.

Click the "Request" field and give a brief title for your request.

Click the "Building" field and choose your respective building.

Click the "Location" field and type the room number or location where your issue is.

Click the "Description" field and leave a brief description describing your issue.

If you have any attachments you would like to include in the ticket, please click on the paperclip at "Attachments".
