    If you have been selected to serve on a faculty member's committee for a Post-Tenure Review (PTR) process, the UTHSC Office of Faculty Affairs will contact you via email to share a unique link for you to complete the required training. The following steps will walk you through completing the training and submitting your attestation.
    After opening the unique link sent by UTHSC Faculty Affairs, click "ACCEPT" to access your training module and attestation.
    First, collapse "PTR Committee Member Nominations - Faculty" step, you will not need this information.
    Collapse "PTR Committee Member Nominations - Department", you will not need this information.
    Read the instructions at the beginning of your form.
    To review the training module, select the "Committee Member Post-Tenure Review (PTR) Training" link.
    ITEM ONE: ATTESTATION OF REVIEW OF POST-TENURE REVIEW (PTR) MATERIALS After reviewing the training module, answer the questionnaire.
    Answer Question 1 by selecting a response from the drop-down menu.
    Answer Question 2 by selecting a response from the drop-down menu.
    Answer Question 3 by selecting a response from the drop-down menu.
    FINAL ITEM: SUBMIT FOR ADMINISTRATIVE REVIEW After the training module has been reviewed and the attestation of completion questions have been answered, you are ready to submit your materials to UTHSC Faculty Affairs.
    In the top right corner of the screen, select "SUBMIT". At any time, you can use the save option to lock in your progress.
    Select "OK" to finalize your submission.
    If you would like to save a copy of your responses for your own records, click "DOWNLOAD A COPY" after submitting. UTHSC Faculty Affairs or the PTR Committee Chair will be in touch regarding next steps.
