ITEM ONE: UPLOAD SELF-NARRATIVE

Describe your milestone achievements and accomplishments for each of the last six (6) years (or since the last PTR review) as well as goals for the next 6-year PTR review period. The narrative cannot exceed two pages.

Upload a master copy of your CV. If you would like to use the UTHSC CV generated from Digital Measures, visit the "Reports" page on the top navigation bar to access the report template. Select the date parameters and select "Run Report", make any necessary changes, then upload in the field below.

ITEM THREE: SELECT PRIOR ANNUAL PERFORMANCE AND PLANNING REVIEWS (APPR)

Select to include the Annual Performance and Planning Reviews (APPR) from the 2020-2023 review cycles. The department chair will upload the APPRs for 2019 and 2017-2018. The prior six (6) review periods are a requirement of PTR.
