Click your user name on the top menu bar

Click "Administration"

Click "Forms & Certificates"

Click "**invoice_v2**" (or the appropriate form that you use)

Using the side slider, scroll down to end of the invoice.

Click below the "[patient_reminder_list]" text to position typing and

Type: "Please answer one question at [www.ReviewYourNameHere.com](http://www.ReviewYourNameHere.com)" (replace with your URL created by us)

Click "Save Form" button at the very bottom.

Click "HOME". Verify by prining a trial invoice and testing the URL
