Add a Question

Enter your username and password and click "Login"

Select Sigma

Click "Administration"

Select your efficacy audit

Click the efficacy questionnaire

You will see a list of questionnaires within your efficacy audit. These are sets of questions

Click "**?**" to view the questions within a questionnaire

Click "Add Question"

Enter the question

Enter the question description / definition. This information will be displayed alongside the question and can include any additional details, such as criteria required for the answer to the question to "pass"

Click to mark the question as a cleaning question or not. After the audit you'll see an overall score for cleaning questions

Select the question aspect. After the audit, you'll see a score for each aspect

Choose the question type. This controls the type of answer required. The options are: - **Yes/No/NA** - the responder will choose from "Yes", "No" or "N/A" - **Option List** - the responder will choose an answer from a list of options - **Number** - the responder will be prompted to enter a number - **Text** - the responder will be prompted to write their answer in a text box

If you choose "Yes/No/NA", choose the default selection. This is the default answer to the question

If you choose "Option List", you will need to add the required options. Click "Add Option"

Enter the option

Choose whether the option is the default answer to the question

Repeat for all required options

Click "x" to delete an option
