The first step to adding custom questions is to create a custom question survey.

To begin the process of creating a custom question survey, click on **Custom Questions** in the upper toolbar. Then, click **Custom Questions Surveys**.

To create a new custom question survey, click on the **Create New Survey** button on the right-hand side of the page.

Add a title to the survey and add a description if you choose. Click **Save** to advance to the next step.

To begin adding custom questions, select the **Add a Question** button.

To create a completely new question, click **Create New**.

In the Choose Question Type menu, select the question type, then **Continue**.

In the box labeled **Question Text**, type your question. For questions with choices, you must also add **Response Options**, which is where you will put your scale.

When you are finished, click the **Save & Add** button in the upper right corner of the page.

Repeat the previous step to add as many questions as you would like.

When you’re finished, select the **Done** button in the upper right corner of the page.

The next step to adding custom questions is attaching your custom questions survey to the project you are wanting to add the questions to.

In the upper toolbar, click **Custom Questions**. Then, click **Custom Questions Projects**.

To add questions to the project, select the plus button below **Add Questions** on the far right.

To add an existing custom question set to your course, select the **Add Custom Question Survey** button towards the top of the page.

You will now see any existing custom question surveys that you have. Pick which set of questions you want to add to a course by:

1. Clicking the checkmark under **Select** next to the custom question\
   survey.
2. Then, clicking **Select Courses** at the bottom of the page to pick which course(s) to add the questions to.

Now, on the Select Courses for Custom Question Survey page, select the **Add Courses** button to open the list of courses to choose from.

A list of the courses you are teaching in this project (this semester) will appear.

Choose the course(s) to add your selected question set to by clicking the checkbox next to the course code and clicking **Add Selected Courses** at the bottom.
