Log In

Log in to the member portal using your established email address and select **Next**.

Enter your password and select **Next**.
Our app and member portal use the same account information, therefore any password changes on the portal will transfer to the app or vice versa.
Registration Steps

To create an account, select **Don't have an account? Register Now**.

Fill in all required fields: First Name, Last Name, and Date of Birth
Choose either your Social Security Number or your Member ID and enter the information in the space provided. Select **Validate**.
**Please note:** Member ID is the preferred identifier for registration. Social Security Number can only be used if this information is provided to Capital Rx from your employer or eligibility provider.

Once on the Create Account screen; enter your email address and your preferred password. Your password must be 8 characters long and include a number, a special character, an upper-case letter, and a lowercase letter.
Select **Create Account** to finalize the process.
You will receive a verification email once you have finalized the setup.
Forgot Password

If you forgot your password, select **Forgot password?**.

Enter your email address and your member ID. Select **Next Steps**.

A secure link and code will be sent to your email address. Click the link in the email to reset your password and enter the code to confirm your account.
My Rx Dashboard

Once logged in, you will land on the My Rx Dashboard. On this page you can view your deductible, out-of-pocket, and plan spend information. For both Deductible and Out-of-Pocket, you can view the amount spent in the plan year, along with the amount remaining if a maximum applies to the benefit by clicking on the graphic to toggle.

If applicable, you can toggle between individual and family plans.

If you are the cardholder for the account, a list of dependents will be available when **Show Family** is selected.

On this page you have on-demand access to your pharmacy benefits prescription insurance card. You can print your ID card by selecting **Print Card**.
If you need customer support, you can call Capital Rx by dialing the number located under Plan Details.

You can access information about your Mail Order Pharmacy and Specialty Pharmacy by selecting the **Forms and Documents Page** link.
Registering with Costco Mail Order

Navigate to <https://www.costco.com/pharmacy/home-delivery> and select **Get Started**.
