After you **Publish** the Google Form, click the **Link** button

Click the **Copy** button to copy the URL address

Open a **new tab** and **paste** the **URL address** to open the Google Form

In the upper-right corner, click the **3-dots**... click the **Cast, save, and share** option

Click the **Create QR Code** option
6
Click th **Copy** button to copy the QR code image (or Download the image)
7
**Paste** the **QR code** image into a Google Doc, Slide, website, etc.
