Select the "**Service**" - (Support Location) of the person supported from the drop down menu

Then click "**Authorize Service**"

Click on the name of the person supported from the SAFE System - or you can use Browse People.

Then click the "**Medical**" tab

Now click the "**Clinical**" tab

Click on the "**Add New**" button

Click on the calendar image to have the Calendar appear.

Now select the date of the medical appointment

Typically you will click "**Yes, Confirm**" -- however, if the Clinical Report is for a hospital stay, click "**No, Skip Step**" and leave the "**End Date**" blank until the visit is over.

Next you will choose the "**Visit Type**" from the drop down menu

Now, select where the medical appointment took place from the "**Hospital or Clinic Name**" drop down menu

Next - you will use the "**Attending Health Professional Type**" drop down menu

Finally -- you will select the name of the medical professional from the "**Attending Health Professional**" drop down menu.

Now - you will enter the reason for the visit -- this information is visible on the Clinical Visits Home Screen -- so please be clear and descriptive.

Next - you will fill in the results of the visit. This is an extremely important section please ensure all information is copied accurately from the attending medical professional.

If there is any ongoing treatment please indicate it here.

If a second staff attended the appointment with you -- you can select their name from the "**Staff Present**" drop down menu

If anyone else was in attendance -- please indicate it here.

Once all of the above information has been entered correctly, click the "**Save**" button -- you're done!
