Click "customize" in the upper right hand corner.

Click "Single-select", "multi-select", "date", or "people". 

*Custom field notifications do not work with text, number, or formula fields. 

Name the field and fill in the options depending on the type of field you have chosen.

To ensure notifications based only on that field, click the "Notify task collaborators when this field's value is changed".

If you would like this field to be available to others in your organization for their use, also click "add to your organization's field library". If you do this, also check "only you can edit their field's settings" so it isn't altered from your settings.
