Click the 'More Options' Toggle and Enter a text on the "Send Button Text" field.

Provide a 'Form Completion Message' and continue to the questions section 

Enter the required Questions and choose the 'Answer Type'. Once the question is completed, click on the 'Save Question Button'

If you want to make the question mandatory, check the 'Require Answer' checkbox and if you would want to get the valid answer, check the 'Require Valid Answer' checkbox

Once the Form is saved, Click on the 'Save Button'
