From the Assessment Tab, click New and choose the "Patient Health Questionnaire (PHQ2/PHQ9)" from the resulting list.

The Questionnaire will open with just 2 questions. If the resulting score is less than 3, you will be finished. Greater than 3 will result in the remaining questions being added.

If the score is >3 then you will be asked if you want to do the full PHQ-9. Answer Yes.

The additional questions will be added. Answer all remaining questions.

Note the "**Total Score" field** once all questions have been answered. Next go to the HPI Tab to record the Questionnaires results.

If you are using the "Medicare AWV" or the "Depression Screen (commercial)" templates found under CC> Health Maintenance, you will see the Depression Screening PHQ-9. Click on this in the Category List to enter resulting Score.

Choose the option on the list that matches the score from the PHQ2/9 that was done. If the score is 0-4, NO DEPRESSION FOLLOW UP PLAN IS REQUIRED.
If PHQ2/9 score is 5+ you must have a Depression Follow up plan documented.
On this example the score was 6.

Scores >5 choose the applicable Follow up plan that will be Prescribed for this patient. For "Additional evaluation ...", "Management of mental Health Treatment" and for "Ordered Referral for Practitioner for depression", all would require a Referral to be ordered. This example used the "Ordered Referral for practitioner for depression".

Choose the Referral Orders from the order tree.

Choose the (RO) REFERRAL as the order.

Open the DPN for the Referral that was ordered by clicking on the blue box.

Choose the Providers and Medical Service Providers in the DPN. You can either click on the "Find" and search the External Provider list and choose which provider you wish to refer to, or if you have a referral coordinator see next step.

Choose the External Provider on the list. When it is added to the field above, in the note field type instructions for what provider you would like this patient to see. Finish fill out any applicable items in the DPN that is needed, then click OK to close.
You have now documented your follow up plan for Depression. Complete note as usual.

Option for Pharmacological Intervention for if you are Prescribing a medication for the patient today for Depression. This can be done in conjunction with a Referral or by itself as an option.

If you are Prescribing an Anti-Depressant for the patient, choose the "Pharmacological intervention (RX'ed today)" from the list.

Go to the RX section of the chart

If this is a new Prescription choose Prescribe to write a new RX. If the patient has been RX'ed a Anti-Depressant previously, it may be listed under Patient Medications and can be refilled from there. This example a New RX is being written.

When the Select Drug screen comes up search for and RX the medication you have chosen. Finish write medication as usual.
You have now documented your follow up plan for Depression. Complete note as usual.
