    Click "Structure"
    Click "Webforms"
    Click "Add webform"
    Click the "Title" field.
    Name your form.
    Click "Save"
    Click "Add element" to add your first field.
    Click the "Filter" field and type "text" to pull up the field "Text".
    Click "Add element"
    In the label field on the pop up, type "Your Name". This will be a text field where users can type in their name. Click Save and Add Element.
    Now type in "select" to pull up the select field. This will be a dropdown field. Click "Add element"
    In element options, you have the option to have a custom, where below you can add your own values for the dropdown. In this case, we will add the pre -made option "Phone type".
    Click the "Title" field and label it "Phone Type".
    Save.
    Click "Add element"
    Add "telephone".
    Click the "Title" field and label it "Phone number".
    Save.
    Click "Add element"
    Add "text area". Text area is a larger type field for users to add a long message.
