Some forms need to be handed in to your school or doctor. For these forms, follow the steps below: fill out the student information, then download and print the form. The information you enter will not be saved on the form.

Click "Medication Form"

Click "Start Filling"

Select your student's grade and fill out the rest of the required questions.

Click "DOWNLOAD AND PRINT"

Click the button at the top right hand corner to download your pre-filled form to your device.
