      Navigate to the Admin Dashboard

      Click "Forms Builder"

      Click "Add New Form"

      Enter the name of the form

      Set the submission window by entering the start and end dates

      **Force Member Login** forces members to login before filling up. This way, submissions are tied to the members' ID and will appear in the Admin Member Info page

      Click **Restrict to single submission** to prevent duplicate submissions, members can only complete the form once. The "Force Member Login." is required to be enabled.

      Enable **CAPTCHA** and **Store Responses** to prevent spam, fake submissions from computer and store all submissions

      For a Yes/No question, choose "Yes/No"

      To create a question with multiple answer options, click 'Multiple Choice'

      Tick this box to require the field

      Leave this field blank or enter 0 if users can choose multiple selections
