In the DASHBOARD view, go to the "MY TEAM(S)" list and click "View" for the selected league/tournament.

Turn on the slider for "Collect Player Documents" and choose the type of document.

Go to "TEAM DOCUMENTS" in the left-side menu.

Here you can determine the "Document Type" and "Document Name".

*If you're a coach needing to collect player documents such as physicals or medical forms, NSID's 'Collect Player Documents' feature can simplify the process. 

When setting up your team, you can either activate the feature manually or it will automatically enable if you're joining a tournament or league that requires it. Once activated, parents will be prompted to upload the necessary documents when joining your team.
