You will also need your child's legal name and Date of Birth, and your proof of income if you qualify for extra hours.

Add your home address or select if you don't have a Colorado address.

Click on your family's annual income range and click "Next."

Select if your child has an IEP and click "Next."
If your child has an active IEP and is receiving services at a public preschool, you won't be able to select a private provider. The special education department will place your child in the program where IEP services will be provided.
