Go to [https://zendesk.com](https://zendesk.com)
Click the “Create a ticket form” button.
Enter a name for the ticket form in the “Name” field.
Select the ticket type from the “Ticket type” dropdown menu.
Select the ticket form position from the “Position” dropdown menu.
Click the “Add field” button to add fields to the ticket form.
Select the field type from the “Field type” dropdown menu.
Enter a label for the field in the “Label” field.
Select the field options from the “Options” dropdown menu.
Click the “Save” button.
