Go to [https://www.paylocity.com/](https://www.paylocity.com/)

Click the "Login" button in the top right corner of the page.

Enter your username and password in the corresponding fields.

Click the "Login" button.

Click the "Myself" tab at the top of the page.

Click the "My Information" tab on the left side of the page.

Click the "Accommodations" tab.

Click the "Request Accommodation" button.

Enter the requested information in the corresponding fields.

Click the "Submit" button.
