Go to [https://www.paylocity.com/](https://www.paylocity.com/)

Enter your username and password in the appropriate fields and click the "Log In" button.

Click the "Myself" tab at the top of the page.

Click the "Health & Wellness" tab on the left side of the page.

Click the "View My Health & Wellness Information" button.
