How to Access and Navigate the Radiant Kids Therapy Patient Dashboard

Go to [https://radiantkidstherapy.raintreeinc.com/dat/PatientDashboard](https://radiantkidstherapy.raintreeinc.com/dat/PatientDashboard) Please enter your email address. If you have more than one email address and are not sure which one we have on file for you please let us know and we would be happy to confirm that for you. If you have forgotten your password you can use the Forgot Password button to have a link emailed to you to set up a new one

For Spanish-speaking families, click on "Espanol Version"

Once you log in, if you have more than one child receiving service with us you can toggle between their accounts by selecting their name near the upper right side of your screen.

Upon your initial login you will see notifications in yellow at the bottom of the page. Please click on "Go" in the blue box to review and sign our new patient forms and to add your child's medical history.

On the "Consent to Release Info..." form you will need to make specific selections before the system will let you move forward.

You will also need to select a specific person

If we already have that info in our system, you can select it to auto-populate the form

Once you have added all of the required info you can click on "...accept this form"

Upon completing all of our forms the system will ask you to Sign them. Once you have added your signature click "Finish & Save" to submit all of your forms.

After submitting your forms please click on "Dashboard" to return to your main Patient Dashboard screen to complete your child's medical history.

From your main "Patient Dashboard" scroll to the bottom of your screen and click on "Go" in the blue box to add the medical history to your child's account

Click "Add Pediatric Medical History"

The first screen "Concerns" is a free-form text box. Please add any/all necessary info

You can move through the rest of the medical history by clicking the list (in yellow) on the left side of your screen.

Once you have completed the form click "Save" on the upper left side of your screen
