    How to Add an Assessment Module (Survey)

    The (Survey) Assessment Module is stocked with features including: -Rating scales -Numeric scale questions -Multiple choice questions -True/False questions -Free form text questions
    To begin, click "Online Training Registration".
    Click "Manage Courses"
    Type in the name of the course you want to add a module to.
    Click "Find Courses"
    Click the title of the course you want to add a module to.
    Click "Add Modules"
    Click "Assessment Module"
    Give the module a name.
    For Assessment Type, Click "Survey"
    Click "Save"
    You'll be taken to the Common Course Module Properties section of the Configuration Tab. There are many different options here.
    For the "Module Name", the name you entered at the previous prompt will appear here, but you can always change it if you wish. This is the name users and admins will see the module as.
    For the "Completion Rule", dictate whether the survey will be required to complete the course or if it's optional.
    Leave the Module Access By Students as the default.
    If there is a due-date for the survey, input that here. If this training will be held as more than a one-time event, you don't want to put a due-date here.
    Dictate if the survey accrues time towards the Minimum Seat Time settings (if applicable).
    Determine the weight (percentage) of the module on the overall course score . If it's optional, this would be 0%.
    You can leave the Session Timeout Setting as the default ("use System Session Timeout") or have a custom session timeout (in minutes).
    Select this option if you want the next module in the course to load upon completion of this one.
