Click "APPLY HERE"
Choose which application applies to you. On the Adult Application and the Adult Guardian application, you will be asked to enter your name and email twice. Please complete both.
Enter your name and email address. (Please note that an email address is required.)
Click the "Your Name" field and enter your first and last name.
Click "BEGIN SIGNING"
Check your email account for an access code. Copy the code into the “Access Code” field.
Click "Show Text" to make sure you copied it correctly.
Click "VALIDATE"
Click "CONTINUE" in the top right corner.
Click "START"
Click the "Required - Enter name of person or organization that referred you." field.
Enter the name of the person who referred you. If no one referred you enter N/A.
Click "Required - Are you still in High School?" Answer "Yes" or "No".
Click the "Required - Enter name of High School being attended." field and enter the name of your high school.
Click "Required - Do you need any accommodations?” and answer “Yes” or “No". Specify the accommodations needed.
Click "Required - Are you seeking sub minimum wage certificate?". Answer "Yes or "No".
Click "NEXT"
The application document will appear. Enter all the information that applies to you.
Click on any needed communication accommodations.
Click the "Required - Name of person who referred you" field.
