    Navigate to [Tally](https://try.tally.so/100days)

    Click "**Sign up**" and follow the sign-up instructions.

    Once you've finished signing up, click "**Create form**" within the dashboard.

    Give your form a name by clicking the form title field and entering your text.\ \ If you're following along with this example, we're going to create a feedback form for students currently enrolled in one of the 100 School challenges.

    While we're here, we might as well add a little customisation to the form by adding our logo. \ \ Click "**Add logo**"

    Click "**Click to choose a file or drag here**" and upload a logo from your device.

    If successful you'll see your logo added to the top of the page.

    Click "**Press Enter to start from scratch**"

    The first thing we're going to do is add a question with conditional logic to ensure that we only receive responses from students that are currently enrolled in #100DaysofNoCode or #100DaysofAI.\ \ Click "**Insert block below**"

    Click "**Questions**". This option allows us to insert a response format with a question.

    Select "**Question + Multiple choice**"

    Click "**Insert**"

    Enter your first question. \ \ In this example we're going to ask users whether they are a student currently enrolled in one of the 100 School challenges. They'll then be served different outcomes based on their response.\ \ Click inside of "**Option 1**" and add "**Yes**"

    Click "**Add option**"

    Enter "**No**" as the second option.

    Click on a new line to create a new block. We're now going to add some text that will only appear when users select "No".

    Enter the text you'd like to be shown to users that are not qualified to complete the feedback form.

    Click the icon highlighted below and select "**Hide**". \ \ What you're doing here is making sure that this block is hidden within the form, until it's triggered to show by user response.

    Click this icon.

    Select "**Add conditional logic**"
