From your Google Account, select Forms or navigate to <https://docs.google.com/forms/u/0/>

Click into "Untitled form" to highlight and delete text

Give the form the name "Simple data form"

You can use the text editor tools to apply Bold, Italicise or Underline to your form name

Click into "Form description" and provide some more detail as to what your form is about

Type "Free guide on how to improve data capture"

You have different question options to select from under the drop-down arrow

Move slider on Required to ensure your capture essential information

Add another question and select "Short answer"

Add question again and select "Short answer"

Click "Question" and type "What is your email address?"

Add one more question, select Multiple choice

In Question, type "Which free guide would you like to receive first?"
