    Complete the following fields: - Email address - Student ID - School name - Zip code (if applicable) - Priority (if applicable)

    Select "Change in student's status" from the dropdown menu for Type of Request. This MUST be selected in order to display additional prompts related to the change in status.

    Choose the appropriate option from the Change in student's status dropdown menu: - Student with related service(s) withdrew from school - Change in related service minutes on IEP - Related service paused - Related service removed from student's IEP

    Select the Related Services Therapy Type for which you are submitting the change in status.

    The service type and minutes fields will display based on the area selected. In the example below, "Counseling" has been selected, so the user is prompted to fill in information related to the change in counseling status. Make the appropriate selection: - Service Removed from IEP - Paused - Student Withdrew - Change in Minutes

    Go to the \[service area\] minutes per month field next and enter the number if minutes (if applicable). If the service has been removed from the IEP, enter "0" (zero).

    Enter a ticket name and ticket description, and click "Submit." If you need to include any additional details, please enter these in the ticket description.
