1. Each Organization is sent one (1) complimentary Accreditation Plaque at the successful completion of their Accreditation/Reaccreditation process.
1. Completion is defined as all programmatic parts of the (re)accreditation process finalized and all (re)accreditation fees paid. These fees include the following:
1. (Re)accreditation Fee
2. Site Visit Fee
3. All Maintenance of Accreditation Fees
4. Trainings/Technical Support/Consulting Fees (if applicable)
2. Additional Accreditation Plaques are then available for purchase at a cost of $165 USD per plaque. This cost covers the shipping and handling costs of the plaque in addition to manufacturing costs.
1. Of note, requests for plaques from a discontinued organization or for a previous Accreditation Cycle will be denied.

4. Payment for Additional Plaques can be made using one of two payment methods:
1. **Check.** Please make the check payable to Social Current and send it to the attention of Social Current, Inc., PO Box 88688, Milwaukee, WI 53288-8688.
2. **Credit Card.** If you would like to make a payment via credit card, please contact Lilibeth Nova in our finance department at [lnova@social-current.org](mailto:lnova@social-current.org).
3. Once Social Current receives both your completed order form and payment, your plaque(s) will be added to our next monthly order to our manufacturing partner.
