Login to Parkland Health's Referral Portal at <https://phhs-referral.eceptionist.com/live/default_login.aspx> To start a new referral, click REFER MY PATIENT.

**Select your referring location from the drop-down.** This list will only populate with your approved locations. You may only see one location in this list. (If your organization has more than one location, and you don't see your location listed in this drop-down, you will need to contact Parkland's Patient Access Center for assistance with your account. In the meantime, proceed with submitting this referral using one of the listed locations. This can be changed later.)

**Select from the service drop-down the specialty clinic where you want to send this referral.**

Click Next

**Clinical Guidelines.** The referral guidelines will appear. Please read carefully and make the appropriate selection. \ \ Click Next to proceed through each screen.

For some specialties, additional clinical requirements may appear. Please read carefully and confirm that your referral will meet the requirements. If you don't believe this referral meets the minimum requirements for submission, exit the referral. \ \ Click Next to proceed to patient information.

**Enter Patient Information.** At this point, you need to provide the patient information. New users who have not yet been granted expanded access, will only be able to ADD PATIENTS to the system and should click the he ADD PATIENT button.

**Add Patient.** Complete the required fields. Be sure to include the patient's mobile# so that Parkland can communicate with the patient about the status of their referral.

Click SAVE CHANGES when done amd Click Next to proceed to referral details.

**Referral Details.** Complete the remaining referral information, including adding ICD-10 codes.

ICD-10 codes can be searched using either the numerical code or the description. Repeat as necessary. Click Next to proceed to the Attachments tab.

**ADD ATTACHMENTS.** Click the ADD FILE button and upload the required referral documents. It is important that all appropriate referral documents are attached as the referral may be returned due to missing or inadequate documentation.

Choose from the Attachment Type drop-down, the appropriate file type for each document that is being attached to the referral.

Click SUBMIT when finished. An attachment confirmation upload may display. Click OK to clear the message.

Congratulations! You have submitted a referral to Parkland Health.
