Select "Contains" as your search option

Insert your organization's legal entity name into the "Search Name" field

Click the box next to "I'm not a robot" for the captcha

Click on the "Control #" for your organization's entity listing

Note your organization's exact legal entity name

Note your organization's Control Number

Click "History"

Note the "Image #" for your "Initial Filing"

As well, if your organization's current name is different than the one listed on your Articles of Incorporation (e.g., Initial Filing) you'll also need to note the "Image #" any Articles of Amendment or DBA/Fictious Name documentation showing the connection between your original and your current entity name
