    Open the form and enter your email address (NOT the therapist's email address).

    Under the "Update Account Information for Therapist/Evaluator" section, check the boxes for all items that apply to the therapist. Additional fields will be displayed based on boxes checked. In the screenshot below, "Name Change" was selected, so fields need to be completed to list the therapist's current name in POC and how their new display name should be listed.

    Enter the appropriate ticket name and description, then click "Submit."

    After the Operations Team has processed the request in POC, the person who submitted the request on behalf of the therapist/evaluator will receive an email notification that the ticket has been closed.

    Alert! If you are filling out the request form due to a therapist resignation or leave of absence, don't forget to also fill out the [Sectioning Request Form](https://share.hsforms.com/1MyeNEC8KQOaEkJF8WDuajQd76zl) to have their caseload transferred to another provider.
