Select "Technology Services Project Request"

Fill in your name, contact number, and department name.

Click on the appropriate division delegate for the project.

Enter a project name that is descriptive but brief. Then write a description that summarizes the project as well as explains the business need and identify any opportunities that could come from the completion of this project.

Click "When would you like this project to go live by? (mm/dd/yyyy)". Then enter the project deadline.

Describe the impact this project will have if not completed.

Click all that apply if the project is a mandatory/regulatory/compliance related project.

Please identify to best of your abilities if the project is a business opportunity, a process improvement, or a value-add to an existing service.

Example: Redefining processes to make them more efficient; Reducing time it takes for a service; Improving cross campus department collaboration.

Select whether the project has funding or not. Then select if there are funds for ongoing maintenance and support.

Click "Save" back at the top of the form.
