    How to Submit an Evaluation Ticket if a Parent/Guardian has Revoked Consent

    Navigate to the evaluation referral form via <https://share.hsforms.com/1pWnNODeDRQ25f-zDRT0R1wd76zl>

    Enter your email address, the student's ID number, and the school that the student attends.

    On the Type of Request dropdown menu, select Change in Student's Status. This will display a field directly below called Change in student's status. Select "Parent/guardian revoked consent for evaluation."

    Enter the ticket name and include any relevant details in the description such as the method parent used to revoke consent and the evaluator assigned. When finished, submit the ticket.
