How to Upload and Edit a form in FormUSign

This document will show you how to upload a document into FormUSign. It includes how to manually add a couple of fillable text fields and a signature to a form that you have uploaded.

From the home page in Tracker, select "Forms" in the blue ribbon bar.

Click "Manage Forms"

Click "Upload New Forms" Note: if you have not created a "Category" you will need to do that first. Unless you want it to be located in Uncategorized.

In the pop up, type in your "Display Name" of the form, select your category from the drop down and then choose your file you wish to upload.

Click "Add form to Repository"

Double-click on the new form to edit

Click the "Text Box" if you are looking to add one for the user to enter some text. In this example I added two of these. If you want the system to scan and find all the fields that could be fillable, choose "Scan for fields". Caution, if your form has many spots that can be filled in, the system will identify them all and you will need to choose which ones are fillable and not.

Move the text box where you want it on the form.

In the far right panel, enter your field name for the text box

Your form will have the text areas where you placed them.

Click "Signature" to add a signature area to the form.

In the far right panel the name of the form label is defaulted to "Sign Here". You can change this if you like.

Click the "Required" if you want the signature to be required. By default, it is set to required.

You can also change the Esignature to add a name and date as well by selecting the little drop down arrow to the right of the box.

Click "Publish" to allow the form to be visible and used for processing. If not, choose close and the form will be saved but not visible to use.

If you are done, you can select "Exit to Tracker" to go back to Tracker and do other work.
