How to Upload forms in Campwise

Login to your Campwise account and select "Login"

Select "Update info" or "Complete Registration" to go to your Parent Dashboard.

Click "Forms"

If your camper will be taking medication while at camp we require a physician signed form. To access that form you'll go to "Download Forms" and select "Download" next to the medication form. It is not necessary to download the other two forms as they're just directions for uploading those two..

Once you have any of the uploadable forms you can go to "Upload Forms". Forms must be uploaded as PDF's. If you don't have them in PDF form you can email them to [office@cubcreeksciencecamp.com](mailto:office@cubcreeksciencecamp.com) or fax them to 573-458-2126

You'll select the form you're uploading from the dropdown.

Select "Choose File" and then select your PDF from your computer.

Then select "upload".

Once it's successfully uploads you should see where it turns green and says "Competed & Uploaded". Once all three uploaded forms and online forms are green you've completed the forms section of campwise.
