If there are any prerequisite courses a "Fill Form" button will appear instead of a "Request" button, click it to fill in the form

If you have already requested a thesis with the same prerequisite courses, your grade will appear automaticly

Click the "I concent that the above information are truthly." field.

Congratulations! You have successfully sent a thesis assignment request to the professor. Both you and the professor will be notified with an email
