How to create a form on a SharePoint site

Navigate to your SharePoint site
Click "Edit"
Click on the "+" icon to add a new content block
Click "+" to add a new web part in your content block
Select "Microsoft Forms"
Click "New form"
Click "Create"
You'll be redirected to Microsoft forms. Click "Add new" to start
Click on the type of form element - Choice, Text, Rating or Date
Edit the form element. For example, add a question.
Click "Add new" to add additional elements to the form
You may choose a theme for your form
Click "Preview" to see how your form looks
Navigate back to your SharePoint tab. Link to your form will be added to the form creator window. Click "OK"
Click "Republish" to save the changes to your site
The new form appears on your SharePoint site
