Navigate to patient's banner bar and click on Create Care Plan button.

Click "Survivorship"

Navigate to the section(s) on the left menu.

Click 'Keep' to keep this section or select 'Reject for this Visit' if this section does not apply to the patient.

Click 'Keep' to keep this recommendation or select 'Reject for this Visit' if this recommendation does not apply to the patient.

Click 'Keep' to keep the task or select 'Reject for this Visit' if the task does not apply to the patient at this time or select 'Reject Permanently' if the task does not apply to this patient.

Click "Proceed to Authorization"

Click "Proceed to Authorization" to authorize care plan.

Sign the document name and Initials.

Click "Accept & Finalize" to generate the care plan.
