      Navigate to <https://www.hotdoc.com.au/dashboard>and select the "**Forms"** tab and select "**Forms"**

      Click "**New Form**" to create the MyMedicare form

      Scroll to the "**MyMedicare Enrolment**" and press "**Select Template**"

      Click "**Next Step**"

      All fields have been automatically copied from the official government [MyMedicare Registration form](https://www.health.gov.au/resources/publications/mymedicare-registration-form?language=en#:~:text=The%20MyMedicare%20Registration%20Form%20is,form%20into%20the%20MyMedicare%20system.) - Review the form and select "**Next Step**" to continue

      If applicable, finalise the form settings:\ \- **Note**: We **do not** recommend linking **the form to a any appointment types**\ \- Add a **Custom message** for patients to view before they fill in the form\ \- Add a link to your **Privacy Policy** and **Terms of use** if applicable\ \ When ready, select "**Create Form**"

      **Please** **do not** **link this form to any of your appointment types** as this is a voluntary scheme, and linking it will mean it is not voluntary.\ \ Instead, we recommend sending a broadcast to eligible patients.

      Your form has now been created and you will need to make it live. Click "**Got it!**" to proceed

      To make this form live to start sending to patients, click "**Make live**"

      Select "**Yes, make live**"to continue:\ \ Please ***do not link this form to any of your appointment types.***

      Your form will then be **Live** for you to start sending to patients.
