    How to setup a Form

    Navigate to [https://app.clickup.com/24323508/v/l/q69dm-3901](https://app.clickup.com/24323508/v/l/q69dm-3901)
    Click on any folder/list you want to add a form to
    Add a form by clicking on '+view' on top of the page & select Form
    Open Form
    You can add multiple forms that suits your needs (bug form, request form, HR form, validation form, ... )
    Choose a form
    There are 2 views in every form: viewing a form & editing a form
    Click 'Editing' when you want to start creating or editing your form
    Choose any of the given fields and drag them onto the form
    Choose for each field if it is required or not
    Delete any field on the form by hovering over the given field and clicking on trash
    There is also the possibility to add custom fields to your form
    Choose any of the given input (location, dropdown, email, phone number, ....) There are a lot of options!
    Create your custom field
    Drag any custom field on the form
    You can simply make any form active or inactive when you want it.
    Users can only fill in active forms. This is a great way of capturing feedback in your desired time frame
    Simply by clicking 'Copy link', you can distribute your form anywhere you want
    There are also a lot of settings you can edit to make sure the shared link is shared the way you like
    Choose where (list / folder) you want your feedback to go
